A number of forms must be completed in order to adopt a Japanese Spitz. Please complete the relevant forms below and return to Japanese Spitz Rescue.
Form 1 - Adoption questionaire that must be filled out by anyone wishing to adopt a Spitz.

Form 2 - Adoption agreement. This contains the terms of adoption and must be signed at time of adoption.

Form 3 - Relinquishment of ownership form that must be filled out by anybody wishing to give up ownership of their spitz and hand them over to rescue to be rehomed
